            Introduction: periodic paralyses (PP) are neuromuscular disorders including hypokalemic paralysis, which can be primary or secondary. Thyrotoxic hypokalemic paralysis is especially interesting because of its low level of clinical suspicion in the Western Hemisphere. Presentation: patient who attended the emergency service in a clinic in Cali Colombia, featuring the classical triad of this entity. After receiving an optimal management his medical condition was fully resolved. Discussion: we emphasized findings in the world literature and their resemblance to the disease clinical presentation in our patient. Conclusion: we emphasize the importance of epidemiological studies analyzing the prevalence and incidence of this disease in our country, as well as, the importance of increasing clinical suspicion for this disorder since it is a medical emergency
