            Introduction: midline cervical cleft represents a rare congenital anomaly of the midline anterior aspect of the neck. Case report:  a 3-months-old patient with no other evident anomalies, successfully treated by complete excision of the visible lesion and cutaneous defect closure with multiple Z-plasty. Postoperative course has been satisfactory with evident resolution of functional limitations and marked cosmetic improvement.
