      Introduction: thyroid storm is an endocrinological emergency with a mortality rate ranging from 20 to 30%. It is a rare condition characterized by a sudden excess in circulating thyroid hormones and multiple organ dysfunction, a consequence of ineffective counterregulatory mechanisms.  Early recognition and prompt intervention can modify the prognosis.  Case presentation: a 40-year-old male patient presenting with a difficult to identify shock, with multi-system dysfunction (hepatic, neurological, hematological), exophthalmos and a Burch-Wartofsky scale score indicative of a thyroid storm. Despite targeted therapy, there was a fatal clinical outcome.
