Introduction: a rectovaginal fistula is an abnormal communication between the rectum and the vagina, clinically manifested by stool passage through the vagina. Case Report: we present a clinical case of a 13-year-old girl who was sexually abused at 6 years of age, who consulted for vulvar air and fecal leakage, despite of having undergone two previous rectovaginal fistula repairs. This time, the rectovaginal communication was corrected through perineal anal-recto-vulvar-vaginoplasty plus a temporary diverting protective colostomy. Discussion: rectovaginal communication repair and fecal continence were confirmed, after surgery, exhibiting excellent cosmetic results for the patient. Some theoretical considerations are made on the different surgical therapeutic options used in girls with this type of acquired lesions. Conclusions: perineal anal-recto-vulvar-vaginoplasty, represents an ideal approach for correcting complex deficits of this nature.
