Introduction: endometriosis is defined as the presence of endometrial tissue outside the uterine cavity, characterized by a chronic inflammatory process, being extrapelvic organs the least affected. It can manifest as a pseudotumor due to its presentation, symptomatology, and imaging and macroscopic findings. Case presentation: a 33-year-old woman with signs and symptoms of intestinal obstruction, mimicking a tumor of uncertain behavior, ultimately diagnosed as ileal endometriosis. Discussion and conclusion: intestinal endometriosis should be considered among the causes of intestinal obstruction, particularly in young women; a high level of clinical suspicion is crucial to insinuate this diagnosis.
