      Objective: to perform an extended review of the literature on anterior cruciate ligament (ACL) reconstruction failure. Materials and Methods: a search of the literature published after 1980, including all types of studies, was conducted using the terms, failed ACL reconstruction. Results: twenty-five studies describing various ACL reconstruction methods and their relation with failed ACL repair were analyzed. A narrative review article on obtained results is presented herein. Conclusion: to determine the etiology of failed ACL reconstruction is important, as the number of primary procedures being performed increases each year and the number of ACL repair is also rising. Correct identification of the cause of failure leads to better outcomes after revision ACL surgery.
