Cutaneous metastases may occur as the first manifestation of an unknown primary carcinoma. Their presentation is rare, they can debut as an infectious, painful picture and are of poor prognosis. The most frequent primary tumors that can compromise the skin are breast, lung, kidney and gastrointestinal tract. The next case is of a patient with metastasis of endometrial origin to the first finger of the right hand.
