The mucoceles of the sphenoid sinus represent 1 to 2% of all those of the paranasal sinuses. A total of 140 cases have been reported, 95% of which occurred in adults, and the youngest was 11 years old. The objective is to present our experience in a seven-year-old patient with a sphenoidal sinus mucocele. Signs and symptoms consisted of scarce purulent headache and rhinorrhea. Diagnosis was made by CT and MRI of paranasal sinuses, as well as by histopathological findings. The treatment consisted of an endoscopic approach. Early diagnosis is important to avoid permanent neurological deficit or visual and nervous alterations by neighborhood. The aetiology, manifestations and treatment of the sphenoid mucoceles were analyzed and the available medical literature is reviewed.
