Resumen

Introducción: se reporta un caso clínico de pielonefritis enfisematosa (PE) en el riñón derecho de una niña de 4 años de edad, con litiasis en pelvis renal de 2.9 cm. Existen pocos casos publicados de PE, la mayoría de ellos en adultos con diabetes mellitus. Discusión: los hallazgos de una hidronefrosis en el ultrasonido renal y una masa dolorosa en el flanco derecho la cual crepitaba a la palpación, hizo sospechar de esta rara enfermedad. La paciente fue sometida a nefroureterectomía derecha de urgencia luego de estabilizar su estado clínico general, el curso clínico posquirúrgico fue satisfactorio.
